They definitely have the different itemids. The problem is that blizzard still bothered to put unique on them, and probably wouldn't want people wearing both versions of them (since both versions are usually good enough to be better than all rivals for the trinkets, and likely for other slots as well. To make things even more confusing, blizzard is not consistent with the tags they're putting on items, and also the datamining sites like mmochamp have some items with a "unique: <itemname> (1)" tag instead of just the unique tag, which sounds like that'd be specifically for these items which share name and stats/effect, but differ in iLvl. However that tag isn't used consistently across all the items. Even worse, the two version of the
same item often have different restrictions. For instance the tanking rings (both alliance and horde):

So you can loot as many of the 245 ones as you want, but only wear one. But if you have a 245 one in your inventory or equipped, can you loot the 258 one at all? Given that it says Unique, you can either loot AND equip it alongside your 245, or you can't loot it at all.
