Intuitive, right click, tracking mod

I need to know the name of an intuitive, right click, buff/debuff/energy tracking mod?

When you loaded it, it started as 3 grey bars. You could click on one of those bars, change the color, input the name of the buff/debuff you wanted to track and then tell it to track you or your target. You could increase the box up to 6 bars to track all kinds of stuff or down to 1 bar and the mod would let you make up to 4 boxes at a time.

The problem is that I don't remember the name of this mod. It wasn't too popular, but it was working as of patch 4.0.
