If they edited the .htaccess file it could be they sshed into the server / exploited another website hosted on the same server and edited all the .htaccess files they could find. Most phpexploits let you take control of the php stuff, but it would have to be a pretty major exploit to let you re-write core files like .htaccess.

On the contrary, the url did change. It kept replacing http://www.failsafedesign.com/maintankadin and maintankadin.failsafedesign.com to russgirs2~

So, facebook went down, Maintankadin got hacked, and apparently steam experienced some problems too(from picture in random picture thread.) Other sites seemed to experience no problems, but these large sites experienced huge problems all at once.

a) it's a Xen virtualized server that Aergis owns so it being from another site is very doubtful (though technically possible, I am not sure what all else is running there, except that it's all under other users so I have no idea how they'd have permission to overwrite the file)
b) not all the htaccess files were edited, just that one
c) the owner/group of .htaccess had changed to the user the webserver runs under
d) wtmp didn't show any questionable logins via ssh or ftp.
