It appears to be at the very beginning of the instance.

this happened to me once as well. (minus the location).
we got malganis down to 1% and instead of him talking/ending encounter he reset to full hp. we said "GG blizz" and proceeded to burn him down again. this time he got to 1% and stayed there. so we tried to drag him out of the area in hopes of finishing him off, no luck.
so we decide to wipe to reset the encounter. stop heals, we die eventually, ghost and run back. come back to see malganis and arthas in an epic duel to the death. both at 1%, neither one dying (as depicted here, i assume). whenever my group tried to kill malganis, he would engage us again, without being able to lose life, and we couldn't do nething about it, nor could we get arthas to die.
we had to page a GM and eventually they started porting all over the instance, and then finally despawned and the chest appeared. was a funny night.

2nd'd, its happened twice now, think it happens when you tank him too close to the stairs behind you, i played around abit and the only times they have bugged is when I tanked him against the stairs...Same thing, 100% - 1% then back to full health then he and Arthas sit and bash each other on 1% hp for the rest of the evening
