Heh
It really depends on what the other people in the group are doing (whether they are focus firing, etc.).
If necessity is the mother of invention, laziness must be the father.

You don't generally conserve mana on boss fights. If I run out I either drink a mana pot (if burst is trivial) or spam rank 1 as soon as I have mana again. By the time the cooldown is up I'll likely have the mana to use max rank again.

Get ktm threatmeter and hope the other party members have it. Boss fights like what others have said, go all out to gain aggro then start getting into a groove if you need to conserve mana. Get some mana pots, they are good for something.

On a boss fight I’ll generally open with max rank and then drop down to rank 4 for most of the fight. If I have enough incoming heals I’ll generally alternate between rank 6 and 4. It all depends on your damage and healing intake. I keep multiple ranks on my bars and decide which to fire when my cooldown comes up based on my current mana pool and threat lead.

i'm spamming the piss out of max rank. rarely am i not getting smashed in the face enough to have a need to downrank.
i find this to be true for the latter half of kara (trash included), also.

Actually Omen/Threat1.0 is far more resource efficient and accurate then KTM. I strongly reccomend everyone dump it. As an FYI it works with KTM for when you group with others who still use it.
Arthas, we're coming for you. (Eventually)
