We had the achievement not launch correctly as well. We did make our first attempt in full gear, but subsequent efforts were at 232 and lower. We're unsure if that is related, or if it's just a completely random bug.

We got it to work correctly this week. The current rumor / speculation is that using ilevel 232 gear in non weapon/offhand slots (IE: the ones available from hard-mode 10man ulduar) causes the problem. Make sure you aren't using 232 sigils or librams; it turns out we had a 232 libram on our first run through.

Again, this is all group speculation, not word from the devs. It does fit the observed behavior for us, at least.

Forgot to post this earlier. It kept bugging on us due to the fact that we had 232's from coliseum the first weeks it was out. Before it was random people having 239's.

We asked a gm and he "suggested" that they may not have added the items from the TotC (5 and 10man). He didn't want to confirm or deny but he said we should try without those items. Next week -> Herald of the Titans.
