With regards to #1, the way I intended it the criteria can be fulfilled as simply as "you must pull your weight in DPS output while the whole raid DPSs the boss, because there is no spare capacity to compensate for you slacking".
This is Patchwerk. Add fights with specific target assignments are just an easier example of this because it is obvious who is failing, whereas in everyone-burn-the-boss personal responsibility is less overt.
