Pretty sure # was originally just meant as a symbol for "number" or "nr./no." as Nooska said. No idea where it first occurred though, but at least been used in accounting for a good while (#3100 as "account number 3100").

Sure, but conceptuall, why would people add Pound and Star to phones? What was the practical application?

They were for special functions. Wikipedia gives an example of British Telecom using them to set up an alarm call.

Yeah, there are a bunch of codes for special things, for example *#100# is how I can check my balance, IIRC. Networks often have a load of codes you can use to get directly to different services using * and #.
